Our Hormone Therapy page offers an optional hormone evaluation quiz. If you take it, we collect whether the quiz is for a man or a woman, your answers to six questions about general symptoms such as energy, sleep, mood and libido, a resulting score, and the name, phone number and email address you provide.

We use this information only to show your result, to email a copy of your result to the address you provide, to notify our care team of your submission, to have a member of our care team contact you about it by phone, text or email as you agreed when you submitted the quiz, and to help prepare for a consultation if you book one. We do not sell quiz information, use it for advertising, or share it with tracking tools.

Quiz information is stored securely with access limited to authorized staff. It is not a medical record, and the quiz is not a diagnosis. If you become a patient, information you share during your care is protected by our Notice of Privacy Practices.

Taking the quiz is voluntary. To have your quiz information deleted, or to stop being contacted about it, call 832.374.0030 or reply STOP to any text message.

Apart from the optional hormone evaluation quiz described above, our website, newsletter sign-up, standard email and text messages are not intended for sharing detailed medical information. Please do not submit diagnoses, medications, lab results or other health details through these channels. Share that information with your provider during your consultation or through the secure methods our team provides.

If you choose to send health information by standard email or text, you understand that these methods may not be encrypted and carry some risk of being intercepted or viewed by others.
